How to Get Started

Disability Waiver Case Management

Minnesota disability waivers (i.e CADI and DD) supports individuals with disabilities who require a level of care similar to that provided in a nursing facility and wish to remain living in their community.

Getting Started:

  1. Complete a MnChoices Assessment through your county or tribal agency.

  2. Determine eligibility for Medical Assistance (MA) and disability waiver services.

  3. Receive approval for waiver services.

  4. Choose a case management provider.

  5. Contact Pathway Support Services, LLC. to discuss your needs and begin services.

Adult Mental Health Targeted Case Management

Adult Mental Health Targeted Case Management helps adults with serious and persistent mental illness access services, supports, and resources that promote stability, recovery, and independence.

Getting Started:

  1. Obtain a current diagnostic assessment from a qualified mental health professional.

  2. Determine eligibility for Adult Mental Health Targeted Case Management services.

  3. Gather any available assessments, referrals, or supporting documentation.

  4. Contact Pathway Support Services, LLC. to discuss your needs and begin services.

  5. If eligible, we can work with you to develop a person-centered plan and connect you to community resources and supports.
